Population Health Management (PHM) – Saudi Arabia

HHH contributed to the transformation of Saudi Arabia’s health sector under Saudi Vision 2030 through the strategic design and implementation of a comprehensive Population Health Management (PHM) framework. This initiative formed a key component of the Kingdom’s health system reform agenda, aimed at improving outcomes, strengthening efficiency, and shifting from reactive care to a proactive, prevention-driven model.

National PHM Strategy Development

HHH supported the Ministry of Health in developing the national strategy for PHM, working alongside a newly established National PHM Expert Group. Together, we co-designed, tested, and validated the foundational tools required for PHM implementation at scale, including:

  • Population segmentation frameworks
  • Case-finding and risk stratification algorithms
  • Health needs assessment tools for demand and supply forecasting
  • Burden of disease and disability assessment methodologies

    These tools form the analytical backbone of the Kingdom’s long-term PHM approach.

HHH designed and implemented the core PHM analytical tools, including:

  • Simulation exercises and predictive modelling
