Population Health Management (PHM) – Saudi Arabia

HHH contributed to the transformation of Saudi Arabia’s health sector under Saudi Vision 2030 through the strategic design and implementation of a comprehensive Population Health Management (PHM) framework. This initiative formed a key component of the Kingdom’s health system reform agenda, aimed at improving outcomes, strengthening efficiency, and shifting from reactive care to a proactive, prevention-driven model.

National PHM Strategy Development

HHH supported the Ministry of Health in developing the national strategy for PHM, working alongside a newly established National PHM Expert Group. Together, we co-designed, tested, and validated the foundational tools required for PHM implementation at scale, including:

• Population segmentation frameworks
• Case-finding and risk stratification algorithms
• Health needs assessment tools for demand and supply forecasting
• Burden of disease and disability assessment methodologies
  
  These tools form the analytical backbone of the Kingdom’s long-term PHM approach.

Data Analysis, Simulation & PHM Tool Design

HHH designed and implemented the core PHM analytical tools, including:

• Population segmentation and risk stratification strategies
• Case-finding algorithms and associated data dictionaries
• Burden of disease estimation models and disability weight tools
• Health supply and demand assessment instruments
• Simulation exercises and predictive modelling
• Pilot Programmes & Validation
• Execution of multiple pilot programmes using real health analytics
• Testing of PHM tools across selected clusters
• Data collection, technical validation, and refinement
• Evidence-based recommendations for national scale-up
• Digital Enablement – Proof of Concept
• A national PHM digital platform was developed as a proof of concept, establishing the technology foundation for PHM data governance, analytics, and future national deployment.

This programme demonstrates HHH’s ability to support ministries and national authorities in designing future-ready, digitally enabled, prevention-led health systems aligned with global best practice and long-term population wellbeing goals.
